Abstract

Surgical therapy of the breast and axilla

Similar to the systemic therapy of breast cancer, also the surgical therapy of breast cancer is currently undergoing significant developments. The necessity of axillary dissection is increasingly being questioned, as study results show that in patients with minor lymph node involvement, the removal of additional lymph nodes does not improve the long-term outcome. The AGO guidelines from 2011 recommend a reduction in ablative radical surgery techniques in favor of skin and nipple-areola complex preserving techniques. In the field of plastic and reconstructive surgery, there is a broad range of surgical techniques. Novel materials such as titan polypropylene nets and acellular dermis further expand the spectrum of reconstructive methods. Implant-based immediate reconstruction is the preferable option in ablative surgery. However, it must be pointed out that the variety of surgical techniques has rarely been studied in comparative studies, which will still need to be addressed.
