Surgical treatment of breast cancer

The objective of breast-conserving surgery is the removal of carcinoma with clear surgical margins.Connected with the radiation therapy, the breast-conserving therapy is a safe method. As surgical techniques the tylectomy/lumpectomy and the partial mastectomy with reconstruction using myocutaneous flap come into question. If a mastectomy has to be performed, for reconstruction there are available alloplastic (expander, implant) as well as autologous surgical techniques (latissimus dorsi flap, pedicle/free transverse rectus abdominis muscle flap [TRAM]).The reconstruction of the areola-nipple complex is mainly performed using local flap plasties and tattooing.

According to most recent findings, the sentinel lymph node dissection can be used also in advanced carcinomas to reduce the morbidity in the shoulder-arm area. CME Prakt Fortbild Gynakol Geburtsmed Gynakol Endokrinol 2009; 5(2): 138-147
