Hysterectomy is still the most performed major gynecological operation all over the world. If uterus size and other conditions allow this, it should be performed as vaginal hysterectomy or laparoscopically. Concerning the decision »supracervical or total« recent overviews show that the preservation or non-preservation of the cervix lead to only minor differences regarding sexuality, pelvic floor and urinary bladder function. After subtotal hysterectomy cyclic vaginal bleedings occurs in 10 % to 20 %. Regarding Treatment recommendations, the presence of a benign indication beside medical factors, the patient's individual circumstances and therapy whishes should be also an issue.
