Progestogens – classification und pharmacology

Besides the natural progestogen, progesterone, there are different classes of progestogens, such as the pregnanes dydrogesterone and medrogestone, 17a-hydroxyprogesterone derivatives (chlormadinone acetate, cyproterone acetate, medroxyprogesterone acetate, megestrol acetate), 19-Norprogesterone derivatives (nomegestrol, demegestone, promegestone, trimegestone, nesterone), 19-nortestosterone derivatives (norethisterone, lynestrenol, levonorgestrel, desogestrel, gestodene, norgestimate, dienogest) and a spirolactone derivative (drospirenone). Some of the synthetic progestogens are prodrugs, which need to be metabolized to become clinically active compounds. Besides the progestogenic effect, which is common for all progestogens, there is a wide range of biological, partial effects, which are different for the various progestogens and has to taken into account, when medical treatment is to be considered and optimal clinical effects to be obtained.
